Terms and Conditions

1. Services
Soma Lactation & Maternal Care provides prenatal and postpartum lactation consulting, follow-up support, and, when clinically indicated, lactation-related medical evaluation and management, postpartum assessment, and mental health screening. Services may include clinical assessment, feeding evaluation, medical decision-making, prescriptions, lab orders, and referrals.
These services do not replace care from your primary care provider or your infant’s pediatric provider.
2. Scope of Infant Care
Lactation services may include observation and assessment of infant feeding. However, Soma Lactation & Maternal Care does not provide comprehensive pediatric care. Clients are responsible for maintaining routine pediatric care and seeking medical attention for infant health concerns.
3. No Emergency Services
This practice does not provide emergency medical or mental health services.
Medical emergencies: Call 911 or go to the nearest emergency department
Mental health crises: Call or text 988, call 911, or go to the nearest emergency department
Communication through email, phone, or website is not monitored for urgent or emergency concerns.
4. Appointments, Cancellations & Fees
Payment is due at or before the time of service
Appointments canceled with less than 24 hours’ notice: $100 fee
Missed appointments without notice: $100 fee
Outstanding balances must be paid before scheduling future visits
5. Insurance & HSA/FSA
Soma Lactation & Maternal Care does not bill insurance directly. A superbill may be provided for possible out-of-network reimbursement. Reimbursement is not guaranteed. HSA/FSA eligibility depends on your individual plan.
6. Communication
Email support may be available for established clients for brief follow-up questions. Responses are typically provided within 24–48 hours. Complex concerns may require a follow-up visit. Email and electronic communication should not be used for urgent issues.
7. Telehealth
Telehealth services may be offered when clinically appropriate and are available only to clients physically located in Minnesota or Wisconsin at the time of the visit. By using telehealth services, you acknowledge potential technology limitations and privacy risks.
8. In-Home Visits
In-home visits are available within a defined service area. Clients agree to provide a safe, smoke-free, and appropriate environment and secure pets during visits. Services may be terminated if conditions are deemed unsafe.
9. Care Coordination
With client permission, relevant information may be shared with other healthcare providers involved in care.
10. Photo & Media
Photos or video may be used for clinical documentation and remain part of the medical record. Use for education or marketing requires separate written consent.
11. No Guarantee of Outcomes
Breastfeeding outcomes vary and are not guaranteed.
12. Limitation of Liability
To the fullest extent permitted by law, Soma Lactation & Maternal Care is not liable for outcomes related to the use of services or information provided.
13. Website Use
You agree not to misuse this website or its content.
14. Governing Law
These Terms are governed by the laws of the State of Minnesota.
15. Changes to Terms
We may update these terms at any time. Continued use of the website or services constitutes acceptance of those changes.
